I'm sure it is very difficult to deliver bad news to someone you don't know. It may be easier if the doctor has some kind of relationship, however tenuous, with the patient or family before offering a prognosis. Or perhaps the doctor could ask, "How much would you like to know? I have information that could be difficult to hear" or something similar. That would empower the patient to receive things more selectively.

But the powerful takeaway from your piece for me is your sister's acceptance and contentment! I think that I'd prefer to know whether my efforts will lead to improvement or not, but I realize that accommodating myself to a life of limitations might not be an easier path. Your sister seems to have a great gift, and I admire it.
